1. Information to caregivers about available services; 2. Assistance to caregivers in gaining access to the services, including information and assistance and case management; 3. Individual caregiver counseling, organization of caregiver support groups, and/or caregiver training to assist the caregivers in the areas of health, nutrition, and financial literacy, and in making decisions and solving problems relating to their caregiving roles; 4. Respite care (in-home, out-of-home [day], and out-of-home [overnight] to enable caregivers to be temporarily relieved from their caregiving responsibilities; and 5. Supplemental services on a limited basis to complement the care provided by caregivers. (No more than thirty percent (30%) of the total federal and nonfederal expenditures for the NFCSP may be used to provide supplemental services.) Examples of supplemental services include: i. Assistive technology, durable equipment, emergency response systems ii. Consumable supplies iii. Home modifications and repairs iv. Legal and financial consultation v. Homemaker, chore, and personal care services vi. Transportation vii. Nutrition services viii. Other - Services must be designed to work in conjunction with other state and communitybased services to provide a coordinated set of supports to family and informal caregivers. - Service parameters (e.g., hourly or monetary caps) for respite care and supplemental services; i. Services will be monitored by the Caregiver Coordinator on a monthly basis and the caregiver will be notified when they are within 80% of the cap. Alternate sources of NYSOFA funding will be investigated as needed. - In order to receive Respite Services or Supplemental Services, both the caregiver and care receiver must reside in Westchester County. - All other services may be provided as long as the caregiver or care receiver reside in Westchester County...
