The clinical pharmacy services, including pharmaceutical services and products, shall be provided to the patients at HSH and adults who are admitted to the State Operated Specialized Residential Program (SOSRP) located on the HSH campus. Per the written request of HSH’s hospital administration, the Provider shall also provide the clinical pharmacy services to HSH patients being treated at contracted psychiatric facilities. The Provider will also provide discharge medications for all AMHD patients, per written request of HSH’s hospital administration, encompassing HSH patients, SOSRP patients, and HSH patients being treated at contracted psychiatric facilities. The clinical pharmacy services encompass the following minimum and mandatory tasks and responsibilities: The Provider shall review the various aspects of the total drug dispensing system of HSH. These reviews will be designed to monitor and safeguard all areas of the drug dispensing system in order to assure safe and rational drug therapy to the patients and to determine the quality and storage of drugs used at HSH. The Provider will have an internal self-auditing program that will be shared with HSH leadership on a quarterly basis. This will assist HSH and the provider to reduce costs and take proactive measures in ensuring sustainability for the program. In addition, drugs shall be purchased directly from wholesalers rather than through the pharmacy vendor to streamline operations, reduce errors, and lower costs by participating in the 340-B program. Failure to conduct internal self-auditing or share findings from the audit by the Provider with the HSH leadership quarterly may result in termination of the Contract. The Provider shall make recommendations for upgrading patient care and improving the quality of life of HSH patients by providing quality clinical pharmacy services. The recommendations shall include drug product selection, formulary compliance, adjusting drug doses and schedules based on the monitoring of the effects of the drug therapies, and cost analysis and effectiveness of selected drug products.
