Services include but are not be limited to, the following: • Provide counseling for an unlimited number of issues per year per employee, dependent or household member. • Current benefit provides up to five (5) counseling sessions per issue per employee, dependent or household member at no cost. If additional counseling sessions are necessary, state cost for additional sessions and/or specify if individuals will be referred to an in-network counselor using their health insurance plan through Cigna (at this point, visits and copays would apply based on coverage under aforementioned health plan). •Current benefit provides the following types of visits:Telephonic, video and face-to-face counseling • Unlimited management consultations, formal management referrals, policy development, and substance abuse screening • Coordination of fitness-for-duty. If there is an additional fee for an examiner, state the amount of fee and how the fee is to be paid (for example, fee will be paid directly by the City to the examiner, etc.). • Unlimited telephonic support for management in the event of a critical incident. Up to 3 consecutive hours of onsite critical incident response to be provided per calendar year. If additional hours are needed, state the amount of fee (for example, fee per hour; or fee per block of hours, etc.); and if there is an additional fee for multiple clinicians and/or bilingual clinicians, state the amount of fee for said clinicians.
