A. Transportation & Documentation i. Provide weekly hauling of materials via a licensed carrier from the designated point(s) of origin to the Permitted Recycling Facility. The points of origin are: a. Salem-Keizer Recycling & Transfer Station 3250 Deer Park Drive SE Salem, OR 97317 b. North Marion Recycling and Transfer Station 17827 Whitney Lane NE, Woodburn, OR 97071 ii. Transport Requirements for Universal Waste Fluorescent Lamps 1. The Offeror will transport fluorescent lamps in secure, labeled containers marked “Universal Waste – Fluorescent Lamps” per OAR 340-113 and 40 CFR Part 273. 2. The Offeror will ensure packaging prevents breakage and mercury release during transport. 3. The Offeror will not require an EPA ID number or hazardous waste manifest solely for transport unless accumulating large quantities onsite (≥5,000 kg). 4. The Offeror will comply with all Oregon universal waste handling and transportation regulations under OAR 340-113. 5. The offeror will comply with all parts of 40 CFR Part 273 6. The offeror will comply with OAR 340-113-0000 through 340-113-0070 iii. Ensure that the transportation of recyclable materials is compliant, as outlined in Section 2, Part 2, section ii, parts 1 through 4, and that the carrier delivers to a permitted recycling facility. iv. Maintain cradle-to-grave chain-of-custody documentation in accordance with Oregon Revised Statute (ORS) 459A.
