Please note: This is a Sole Source Notification. The State intends to make a noncompetitive purchase for the provision of the North Dakota Transition and Diversion Program services to include transition coordination, transition assistance, outreach services, and overall coordination of transition services for individuals with a disability in the Northwest quadrant of North Dakota, including those counties serviced by both the Northwest and North Central Behavioral Health Clinics. The North Dakota Transition and Diversion Program provides transition coordination services to qualified consumers with transition from Nursing Facilities (NF) or other qualified settings back into the community. These services are provided through collaboration with Home and Community Based Services (HCBS) Case Managers, Development Disability Program Managers, consumers/family members, nursing facility staff, and community agencies, as well as through coordination of transition activities leading to a return to the community. The service is also intended to divert individuals at risk of NF, Basic Care, or an Intermediate Care Facility.
