Nassau County, New York (the “County”) is currently seeking proposals from qualified individuals and entities authorized to do business in the State of New York, to provide Family Caregiver Support Services for the Office for the Aging (OFA) The purpose of the RFP is to provide Nassau County with proposals and recommendations for Family Caregiver Support Program to provide supportive services, assist them in caring for their Care Recipient. It should be noted that the National Family Caregiver Support Program under Title lllE funding, provides services directed towards the Caregiver as the primary recipient. The services provided are those that support the Caregiver in performing their duties and responsibilities and those which allow the Caregiver to resume normal daily activities at home and in the community.
