4.1 Mandatory Contract Services Requirements and Deliverables: Contract Services must meet or exceed the mandatory requirements listed below. 4.1.1 Maintenance 4.1.1.1 Response time for repair must include travel to the instrument location. The vendor must notify the department by email within 24hours of receiving the service call of when they are expected to be on-site. The on-site visit must be within 5 business days of the initial service call. 4.1.1.1.1 The instrument is located at 313 Gus R. Douglass Lane, Charleston, WV 25312 4.1.1.2 Must include repair parts at no cost to the Agency other than consumables. 4.1.1.2.1 Consumables will not be purchased through this contract. 4.1.1.3 One maintenance visit annually with a reminder from the contractor for the due date. All maintenance services will be scheduled by the Agency between 8:00am and 4:30pm, Monday through Friday except holidays. 4.1.1.3.1 The vendor will follow the maintenance guidelines provided in the equipment manufacture’s service manuals. 4.1.1.4 Software and Equipment updates are to be completed by vendor who must have certified and trained Technicians specific to the PerkinElmer software (Simplicity™, Empower™, & Turbomass™). 4.1.1.5 Must include In-Person Repair Service as required/needed. This will be included in the annual cost. 4.1.1.6 Must include Phone/Remote Support as required/needed. This will be included in the annual cost. 4.1.1.7 Must include costs associated with Labor and Travel. 4.1.1.8 Detailed service report from Vendor for work performed. 4.1.1.8.1 Vendor’s service report should include but not be limited to 1) initial reason for service. 2) what was discovered during initial review, 3) items replaced and 4) final summary. The report should be sent to Deputy Director or proxy.
