1. Personnel provided by vendor to Lexington 4 upon a contract being awarded shall be qualified to perform the services requested and must be South Carolina certified and/or licensed at the time the proposal is submitted and remain in effect during the term of the contract to perform the applicable evaluation and/or special education clinical services and/or related services in the state in which Lexington 4 requests a clinician to perform services. 2. Personnel provided by vendor to Lexington 4 upon a contract being awarded shall be eligible to submit claims to Medicaid for school- based Medicaid services in Lexington 4 schools that participate in Medicaid billing. 3. Vendors will conduct a thorough background screening of potential personnel to include a federal and state background check, fingerprint clearance, verification of education, work history, and licensure/certification, Tuberculosis screening, and professional reference check, as required by Lexington 4. Lexington 4 reserves the right to check all vendors and/or potential personnel for suspension and disbarment.
