The Agency wants assistance with comprehensively evaluating and improving surveillance for the Mississippi Delta Health Collaborative (MDHC) initiatives. The Contractor will work closely with the MDHC staff to plan, design, collect and analyze data, report findings, ensure ethical standards, manage relationships with stakeholders, and engage in continuous learning and improvement. The goal of this project is to ameliorate cardiovascular health outcomes, specifically heart disease and stroke by improving hypertension and cholesterol control rates in the Mississippi Delta Region. Efforts are focused on the following targeted counties: Bolivar, Carroll, Coahoma, Desoto, Holmes, Humphreys, Issaquena, Leflore, Panola, Quitman, Sharkey, Sunflower, Tallahatchie, Tate, Tunica, Warren, Washington and Yazoo County.
