The required Dental Services are to be provided by a Licensed Dentist for the patients at the Austin State Supported Living Center (AuSSLC), facility location 2203 W 35th St, Austin, TX 78703. Services shall be performed at Austin State Supported Living Center Campus/Dental Clinic, local hospital and/or Contractor’s approved local dental office as appropriate. Dental services including, but not limited to General Dentistry, Endodontics and Dental Anesthesia as needed. Contractor shall provide dental exams, consults, x-rays, provides general dentistry/restorative services including all preventative procedures, tooth extractions, surgical and non-surgical root canal therapy, direct and indirect restorations, fixed and removable prosthetics/devices, dental implant restorations, deep scaling root planning, crown lengthening, biopsies, and pathology evaluations for the dental clinic at AuSSLC. Contractor shall provide general anesthesia, conscious sedation and/or full sedation as needed and shall monitor the conditions of the patient undergoing surgery prior to, during and after surgery. Contractor shall perform a risk assessment of each patient who may require services and provide the appropriate type of anesthesia for the procedure performed, based on the identified risks. Contractor shall meet all requirements of the Texas State Board of Dental Examiners (TSBDE) to provide anesthesia services. Contractor shall provide services as needed before and after surgery. Contractor shall provide services to AuSSLC clients including, but not limited to, providing and reviewing annual exams, evaluations, consultations, and specific dental treatment as appropriate to the patient(s) on campus as needed. Contractor agrees to maintain, clean, and sterilize all dental equipment as needed including all preventative maintenance. Contractor and associated personnel will enter findings, recommendations, and Treatment summary onto a hard copy consultation generated by AuSSLC personnel or provide legible and signed clinical notes. Contractor’s findings shall be entered into the electronic medical record by AuSSLC staff. A copy will be provided to the patient’s Treatment Team for inclusion into the Treatment Plan as needed.
