The selected Sub-grantee will operate a mobile outreach and transportation services program in alignment with the outlined framework. Services must align with the post-shelter transition strategy adopted by the City Council and will address the needs of unsheltered individuals through proactive, trauma-informed interventions. Activities to include: Providing 24/7 trauma-informed outreach and navigation of services. Coordinating transportation to shelter, health care, treatment programs, and family reunification. Verifying client income eligibility. Supporting encampment response outreach and working collaboratively with code enforcement and law enforcement. Providing quarterly City Council reporting summarizing services, referrals, demographics, and barriers. Documenting client interactions, eligibility, and outcomes securely.
