As described in the Purpose statement above, allowable services and criteria for eligible applicants are outlined in 42 USC 669b. Applicants must be able to provide one or more of the allowable services. The initial contract period will be from October 1, 2025 to September 30, 2026. Local service providers will be required to submit monthly A-19 invoices with supporting billing documentation detailing hours and services provided for each individual receiving AV program services to the DCS Grants Administrator. Local service providers will also be required to submit quarterly and annual survey data reports to the DCS Grants Administrator. The DCS Grants Administrator or their designee will conduct site visits during the period of performance. Bidder shall identify the key personnel it shall utilize in performing this Contract, and their experience and qualifications, as part of its Response. If awarded a Contract, Bidder shall not make changes to such Key Personnel during the term of the Contract except as requested or approved by DSHS.
