Offerors shall provide their proposed costs below. Indicate pricing for all renewal periods for support and maintenance. Cost proposals must be fixed price and include all costs for the total implementation of the project as described in the RFP. On this sheet, provide your total cost for the entire project. This total price must include all direct and indirect costs associated with the performance of the contract including, but not limited to, all project deliverables, activities, personnel costs, licensing costs, hardware costs, total number of hours at various hourly rates, direct expenses, payroll, supplies, overhead assigned to each person working on the project, percentage of each person's time devoted to the project, profit and completeing tasks, and other services that you propose, to cover all of the requirements in Section 3. Scope of Work and Contract Information. IMPORTANT - Offeror's must use the form to enter data that will be utilized to determine the proposed costs for provision of services. Do not alter this form or add additional information as it is used for evaluation purposes to convert the costs to points. Rates shall not exceed the tenth decimal place.
