1. Performs in-depth, nutritional assessments upon admission and quarterly, annually, and when a significant change occurs for the Veterans of BGVC. Maintains accurate documentation and tracking of Veteran’s medical nutritional therapy and nutritional risk e.g. weight loss, thickened liquids, wounds, feeding tubes. 2. Documents nutritional care plans and makes written evaluations and maintains a specific care plan for each Veteran updating to reflect ongoing changes of Veteran’s status. Plans and supervises the implementation or therapeutic diets and writes orders as prescribed by Physician. 3. Consults with other health care professionals for Veteran’s medical nutritional therapy. Participates in care plan meetings, Veteran care conferences, and committee meetings to address all nutritional concerns. 4. Interprets and ensures that the Veteran receives the recommended dietary requirements. 5. Assesses the compliance of food service operations with all state and federal rules and regulations.
