A. Contractor shall provide to Caltrans all labor, equipment (including rentals), tools, supplies, licenses, permits, travel, fuel, mileage, materials, parts and incidentals necessary to inspect and repair the roll up doors at the locations listed in Item 3, above and on Attachment 3, List of Roll-Up Doors/Gates with Dimensions and Motor Types. Work shall include but not be limited to: 1) Contractor shall inspect motor, belt and chain operation. 2) Contractor shall inspect switch and button operation. 3) Contractor shall inspect springs for tension. 4) Contractor shall inspect all rollers for proper alignment and operation. 5) Contractor shall inspect all panels and hinges for straightness, alignment, and proper operation. 6) Contractor shall inspect all track channels for straightness and alignment. 7) Contractor shall inspect all safety edges for proper operation. 8) Contractor shall inspect and adjust limit switches. 9) Contractor shall lube and adjust all parts as necessary. 10) Contractor shall repair and replace parts as necessary. 11) Contractor shall inspect all electrical parts and wiring, touch pad controls, transmitters, radio receivers, keypads, knox box, ground loops, eye sensors, chains, wireless edge, bollards, wheels, v-track, gate operators and any related components to electric doors and gates. B. Contractor shall list all necessary repairs and submit the list to Caltrans Contract Manager for approval before starting any work. This applies to all repairs, including those requiring emergency attention at any time, to ensure uninterrupted service delivery. Once approved by Caltrans Contract Manager, Contractor shall schedule the repairs according to the timeframes listed in Section 5 above. C. Contractor shall start and complete work at the designated job site(s). Commute time to and from the job site is not chargeable, as all travel-related costs shall be included in the Unit Prices listed in Attachment 1, Price Quote Proposal. D. Contractor shall not perform any repair work estimated to exceed $500.00 without obtaining verbal authorization from Caltrans Contract Manager. E. Contractor shall not perform any repair work estimated to exceed $1,000.00 without receiving written approval from Caltrans Contract Manager. F. Contractor will be called back to repair or replace any work or material deemed substandard by Caltrans Contract Manager or found to be out of compliance with any applicable codes or permits. All call-back repair or replacement work shall be performed at Contractor’s sole expense. G. If the services performed by Contractor are not acceptable by Caltrans Contract Manager after a third call-back, or on multiple jobs, Contractor shall be advised that the non-compliance will constitute grounds for termination in accordance with Exhibit D, Termination. H. Contractor shall inform Caltrans Contract Manager immediately in the case of delays in procurement of spare parts and assemblies. This shall allow changes to be made to effect the most feasible and suitable repairs. I. It is understood and agreed that any and all work assigned pursuant to this Agreement is to be performed in accordance with this Agreement. In no case shall Contractor accept Service Requests directly from tenants, lessees, or other contractors. J. Caltrans will pay only for work requested by Caltrans Contract Manager. Caltrans Contract Manager will notify Contractor of the location, nature, and extent of the work to be done. The service call and Service Request shall be the complete instruction and authorization for the proposed work and Contractor shall comply fully with all contract requirements. K. Contractor shall only perform the work authorized on the Service Request. Provided the subject matter of the Service Request is not changed, the Service Request may be amended by mutual written agreement of Caltrans Contract Manager and Contractor. If no contemplated work is required to accomplish the intent of the order, additional authorization shall be obtained from Caltrans Contract Manager before said work is begun; in other words, additional work cannot begin until the Service Request is amended accordingly. L. In a written document, Contractor shall list the inspections and repairs that were performed, and briefly describe the inspection results and repairs. M. Caltrans Contract Manager will request, through a Service Request, all repair work covered under warranty to be done by Contractor. N. Caltrans Contract Manager will typically request maintenance services on an annual basis. The existing doors are approximately 20 years old and the openers range in age from 1 to 10 years. O. Contractor shall submit receipts for materials and provide before-and-after photographs of the work performed with each invoice. Invoices submitted without the required documentation will not be processed for payment.
