Specifications include, but are not limited to: The required services to be provided are for Therapeutic Drug Monitoring of Anti-Tuberculosis medications for patients cared for by the Texas Department of State Health Services (DSHS). Services to be performed: • Testing will be provided at costs of services to the TB and Hansen’s Disease Unit based upon Contractor's fee schedule as it is negotiated with the Unit. • Testing services will include the cost of supplies, result delivery, and statistical reports. • Contractor agrees to render therapeutic drug monitoring services for the patients of the Unit’s designees in accordance with orders given by physicians of said patients. • Contractor will perform TDM tests and report results to the submitter location. Provide the Unit with the following test information: • Laboratory name and address. • Laboratory phone and fax number. • Medical Directors name and phone number; and email address. • Test offered, including test name, pricing, CPT/HSPCS code, and specimen requirements. • Contractor will provide consultation to the submitter if needed to clarify interpretation of results. Provide monthly reports to the Unit regarding the following: • List of all senders of drug levels by sender name and city/county site and tests ordered per site. Results of each of the tests performed. Perform all test ordered by the Unit’s designees. All test orders must be accompanied with an Infectious Disease Pharmacokinetics Laboratory (IDPL) requisition with the following information: patient last/first name, date of birth, patient Identification (lD), referring physician, facilities fax and phone number, drug name to be assayed, International Classification Disease (ICD) ICD-9 Code, drug dose (mg) and specify PO, IV, IM, number of doses per week, date of last dose, time of last dose, date blood drawn, and time blood drawn. • Deliver a copy of the original laboratory report in a timely manner to the senders (requestors of the tests). The laboratory test report will include at the minimum: patient's name, date of test, test name, test results, normal values, laboratory name and address. Contractor agrees to make all Unit designee patient records to whom the Contractor has rendered services for available to the Branch’s inspection upon request. Provide the following Specimen transport and Courier services: • Transport & Courier: Transported specimens must be packaged and handled by local and regional TB programs supported by DSHS according to IDPL Shipping and Handling Guidelines and according to OSHA Guidelines. All samples will be shipped by the local/regional TB program via overnight delivery to arrive Monday through Thursday.
