The required services to be provided are Dental Services including, but not limited to General Dentistry, Oral Surgery and Periodontal services for the residents served by the Denton State Supported Living Center, located at 3980 State School Road, Denton, Texas 76210. Services shall be provided on an as needed basis and may be added and or removed during the term of the contract. Services to be provided at Texas Health Presbyterian Hospital located at 3000 I-35, Denton, Texas 76201 and/or Contractor’s local dental office as appropriate and requested by DSSLC. Contractor must be affiliated and credentialed with Texas Health Presbyterian Hospital in Denton, Texas. Contractor must have an office located within twenty (20) miles of DSSLC. Contractor must have experience with Intellectual Developmental Disabilities (IDD) patients. Contractor must have 3 to 5 years’ experience working with State Supported Living Center (SSLC) clients and IDD clients.
