It is the intent of these specifications, terms and conditions to describe the subject matter expert (SME) consultation and related services as required by the Alameda County Health (hereafter AC Health or the County). The County seeks to contract with SMEs that have experience providing services to Medi-Cal beneficiaries, Medi-Cal eligible individuals and/or vulnerable populations, or systems that support them, to provide consultative services, as needed, to the staff and stakeholders of AC Health. AC Health has an immediate need for SMEs to consult for its California Advancing and Innovating Medi-CAL (CalAIM) - Community Supports and Enhanced Care Management Project, described below, but may require services for other programs and departments throughout AC Health. These SMEs will act as resources, guides, coaches, technical assistors, and implementation partners for the planning and execution of a wide range of care coordination and other support services for AC Health and its partners.
