The Minnesota Department of Veterans Affairs (“MDVA”) requests proposals to provide a full range of occupational therapy, physical therapy, and speech therapy services to the Minnesota Veterans Home - Montevideo; Specifications include, but are not limited to: Sample Tasks and Deliverables. 2.1 Provide a full range of occupational therapy services, including but not limited to treatment for lymphedema, venous edema, and other conditions requiring specialized therapy to residents onsite at MVH-Montevideo on a mutually agreed upon time schedule. 2.2 Provide Occupational Therapist and Certified Occupational Therapist Assistant (“COTA”) services as needed. 2.3 Provide a full range of physical therapy services, including but not limited to evaluations, treatment for mobility improvement, falls prevention, and pain control to residents onsite at Home on a mutually agreed upon time schedule. 2.4 Provide Physical Therapist and Physical Therapist Assistant (“PTA”) services as needed. 2.5 Provide a full range of speech therapy services, including but not limited to: speech/linguistic therapy evaluations and treatments, swallow evaluations, and treatments for modified diets and high-risk residents as needed for events/occurrences to residents onsite at Home on a mutually agreed upon time schedule. 2.6 Provide written evaluations and recommendations to Home regarding current condition, rehabilitation potential, and rehabilitation goals for all residents provided services. 2.7 Provide consulting services at request of Home, including participation in resident care conferences, development of appropriate restorative therapy policies and procedures, and attendance at quality-of-care reviews and committee meetings, as appropriate. 2.8 Document and recommend appropriate treatments and instructions that address residents’ needs and promote the highest level of resident function. 2.9 Provide qualified professional personnel to perform the services described in this solicitation document. 2.10 Provide evidence of sufficient malpractice, liability, and other insurance as specified in the sample contract Exhibit B, upon request by the State.
