Activity 1: • Outline your maintenance plan for the mobile medication unit. • Describe how the funding of the maintenance of the mobile medication unit will increase access to services and improve patient retention. • Describe how the funding of the maintenance of the mobile medication unit will assist in longterm sustainability of the unit. Activities 2, 3, and 4: • Demonstrate that peer services will be based at one office that holds a Wis. Admin. Code §§DHS 75.50 and 75.59 or Wis. Admin. Code §DHS 75.60 certification, including branch offices under a respective certification, and how the services will operate within the borders of Wisconsin. Describe how your branch office (if any) meets the certification requirements. • Describe the approach to plan for the development, implementation, and integration of Peer Services in OTPs and/or OBOTs. This includes supervision of peer support services. • Describe the proposed program that will utilize peer support services. • Describe how the proposed program will increase access to services and improve patient retention. • Describe how the proposed program or project will address workforce development including, hiring, training and retention of peer workers. • Describe the supervision needs of peer workers and how the proposed program will provide effective supervision for peer workers. • Outline the training timeline for supervisors of peer support workers to have the supervisor fully trained within four months of award. • Describe the proposed hours that peer support services will be available.
