• The provider must attend and provide a program status update at 75% of all Cumberland County Human Services Advisory Council (HSAC) monthly meetings held during the contract year. • The provider must participate in annual program/site monitoring to ensure the program is functioning properly and in compliance with all requirements. • No amendments to the approved funding program design as outlined in the contract may be made without approval by the HSAC and/or the HSAC Coordinator. • As requested, provider agrees to present a copy of audit reports and client files concerning this contract. • As a joint effort between the provider and the HSAC, any press or media contact in relation to this contract must be coordinated in advance. Additionally, the contractor shall be required to post an acknowledgment in all advertisements of the programs funded through the Cumberland County Department of Human Services/Human Services Advisory Council, the acknowledgment shall include the logo of the County of Cumberland. The contractor is required to submit all advertisement materials to the Department of Human Services for approval prior to issuing materials to the public. • Programs that are open to the general public shall create a provider login at nj211.org, and maintain accurate and updated listings in the 211 directory. Programs shall review and update their funded program listings in the NJ 211 directory semi-annually, at a minimum, and whenever there is a significant change in program offerings, availability, or contact information.
