A. Comply with HHSC State Supported Living Center/Facility Uniform Terms and ConditionsVendor, which are incorporated as part of this Purchase Order as Attachment A. B. Provide HHSC with the following service(s) to patients of the Facility: • Provide all oral surgery, extractions (including 3rd molars), implants, evaluations, retained root procedures and provides biopsies and pathology evaluations. • Provide dental exams, consults, x-rays, general dentistry/restorative services including all preventative procedures, tooth extractions, surgical and non-surgical root canal therapy, direct and indirect restorations, fixed and removable prosthetics/devices, dental implant restorations, deep scaling root planning, crown lengthening, biopsies, and pathology evaluations. • Licensed Dentist will provide services including, but not limited to, providing, and reviewing annual exams, evaluations, consultations, and specific dental treatment as appropriate to the client(s).
