The Contract shall provide hands-on physical examination that must include the following mandatory areas: 1. Labs & Urinalysis to include: a. Urinalysis b. Complete Blood Count c. Complete Metabolic Panel d. Lipid Panel with LDL/HDL Ratio e. A1C f. Thyroid Stimulating Hormone g. Prostate Specific Antigen (male patients 40 and over) h. FIT Fecal Occult Screening Kit (patients 40 and over) i. Immunizations and Infectious Disease Screening j. Post-Exposure Bloodborne Pathogen Testing k. Titer checks as indicated 2. Full Physical to include: a. Comprehensive Physical with Health History & Vitals b. Titmus V2 Advanced Vision Testing c. Audiometry Testing d. Skin Cancer Assessment e. Behavioral Health Assessment f. Sleep Disturbance Screening g. Cancer and Cardiovascular Disease Risk Reduction h. Occupational Stress Awareness Consultation i. Hormone Imbalance Awareness Consultation j. Behavioral Health Screening k. Weight and Body Composition l. Review of Results w/ Personalized Health Plan 3. Cardiopulmonary Screening & Fitness Assessment to include: a. Cardiopulmonary Exercise Testing (CPET, consisting of 12-lead EKG treadmill/stepmill stress test run to maximal volition utilizing WFI protocol, Blood Pressure readings, and TRUE VO2 Max Testing) b. Pulmonary Function Testing (Spirometry) c. Metabolic Analysis w/ Body Composition d. Muscular Endurance e. Strength and Flexibility Evaluation f. Cardiology follow up interpretation for irregular results, if indicated g. Chest Radiographs-as recommended 4. Cancer Screening: a. Ultrasound imaging of the carotid arteries, thyroid, liver, pancreas, gall bladder, spleen, kidneys, bladder, pelvic and breast (women), testicular and prostate (men) b. Colon Cancer Screening c. Prostate Cancer d. Lung Cancer Screening e. Cervical Cancer Screening f. Testicular Cancer Screening g. Bladder Cancer Screening h. Oral Cancer Screening i. Thyroid Cancer Screening j. Skin Cancer Screening k. Radiology follow up interpretation for irregular results, if indicated 5. Heavy Metals Profile I, Blood Test a. (arsenic, lead, mercury) 6. Galleri 50+ Multicancer Detection Test: (Optional) 7 Employees 40+= Once every 5 years, 50+= every year Galleri checks more than 100,000 DNA regions and over a million specific DNA sites to screen for a signal shared by cancers that could be hiding. The Galleri test looks for cell-free DNA and identifies whether it comes from healthy or cancer cells. One DNA from cancer cells has specific methylation patterns that identify it as a cancer signal. Methylation patterns also contain information about the tissue type or organ associated with the cancer signal to guide the next steps. 7. Medical Consultation Each employee shall receive a one-on-one review with a licensed medical professional (MD, DO, or NP) to discuss all aspects of their physical exam, including blood work and other test results. In the event of any irregular findings, the medical professional will recommend a follow-up process. No medications will be prescribed or administered without the consent of the employee’s primary care physician.
