A. Required Services ● Provide direct and consultative occupational therapy services as identified in IEPs and 504 Plans. ● Deliver all services in Decatur City Schools locations unless otherwise directed by the Supervisor of Special Services. ● Collaborate regularly with Decatur City Schools’ OT staff and communicate with classroom personnel. ● Participate in eligibility, IEP, and 504 meetings as needed.d ● Complete session notes, progress reports, and Medicaid documentation (if applicable) B. Data Access ● All documentation must be entered and maintained in a secure, web-based system accessible to designated school personnel for real-time monitoring. C. Confidentiality & Professional Conduct ● Comply with all student confidentiality requirements under FERPA and HIPAA. ● Maintain a valid Alabama Occupational Therapy license. ● Agree not to divulge confidential student or district information during or after contract termination. ● Abide by a non-solicitation clause prohibiting either party from recruiting the other's staff or clients for 12 months after contract termination without written permission. D. Independent Contractor Status ● Acknowledge that the provider is an independent contractor, not an employee of Decatur City Schools. ● Contractor shall not enter into third-party agreements on behalf of the school district. E. Equipment and Training Support ● Recommend appropriate therapy equipment and materials to support implementation. ● Offer consultation and training to staff regarding classroom accommodations, sensory integration strategies, handwriting supports, and motor planning tools.
