The purpose of this contract is to obtain a sustainable, statewide framework to contain a system of services for the population of focus. Vendors should propose one of the following: A sustainable framework for providing services to the population of focus that already exists, has been implemented in other state(s), and addresses Nevada’s realities, OR A plan to work with the State to develop a sustainable framework for providing services to the population of focus. All proposals should address the following five phases: Phase I: Needs Assessment. Through independent research and discussions with key staff, Vendor will perform a comprehensive assessment of the Nevada landscape of services for the population of focus. The assessment should describe current gaps, barriers, or related challenges, as well as areas of success, and an explanation of factors that contribute to these issues/successes. The needs assessment should include identification and assessment of existing: Pathways: to include access points, referral pathways, decisions/rules for matching children and youth with services (including level and intensity), entrance/exit criteria, and related information. Providers: to include existence of, access to, and maintenance of a roster of participating providers and services, and related information. Engagement: to include typical child/youth/family engagement levels and types throughout the process and related information. Care coordination: to include multi-system navigation, outreach, referrals, progress monitoring, communication across organizations, and related information. Quality management: to include data-based decision-making, fidelity monitoring, quality assurance, continuous quality improvement and related information.
