2.1.1. Identify and inventory of existing services and supports specific to nutrition available to CYSHN, such as Community Health Team, outpatient services and hospital-based programs. 2.1.2. Develop, maintain, & improve the system of care that is collaborative, interdisciplinary and evidence-based. 2.1.3 Provide training for supporting local teams serving children with complex health and developmental needs and their families. Provide ongoing technical assistance to these teams. 2.1.4 Promote availability of consultation services to providers, specifically those providing nutritional services to CYSHN. This should be conducted in collaboration with FCH Information Director. 2.1.5. Provide technical assistance and consultation to approximately 25 providers/teams caring for CYSHN. 2.1.6. Technical assistance activities may include: (a) modeling and coaching of strategies and interventions; (b) sharing of resources referencing best practices and current research; (c) identification of risk factors or indicators that may warrant additional referrals; and (d) collaboration with CIS and medical providers and agencies. 2.1.7. Consult on medical necessity and clinical eligibility for nutrition services for CYSHN when referrals or requests for services are made to be covered by Department of Vermont Health Access (DVHA), also known as Vermont Medicaid. 2.1.8. Provide between 4-6 regional trainings for CIS providers, preschool providers and Vermont families through face-to-face or distance technology based on identified statewide, regional, and local needs. Areas of training may focus on regionally identified themes or topics (e.g. Common nutritional issues in children with Autism Spectrum Disorder) or nationally recognized best practice(s) or advancements. Trainings that are local and child focused based on a specific diagnosis or team need will be considered technical assistance rather than counting as a formal training. 2.1.9. Engage in collaborative problem-solving around specific needs of this population with representatives of statewide agencies such as Vermont Agency of Education, Agency of Human Services: Children’s Integrated Services (CIS) - Early Intervention, Early Hearing Detection & Intervention, Medicaid, and Children with Special Health Care Needs (CSHN). 2.1.10 Collect and provide feedback from consultation recipients and teams supported through contract work.
