A. Client Eligibility for Receipt of CHOICES-funded Health Care Services Participants must meet and maintain established eligibility criteria to receive healthcare services through the CHOICES Program: 1. Citizenship and Immigration status: United States citizenship through birth or naturalization, or a permanent legal resident. 2. Alachua County residency – CHOICES funding is to be used exclusively for residents of Alachua County. Individuals or families residing elsewhere may not enter the county for the purpose of participating in a CHOICES-funded program. 3. Age – 18 to 64 years of age, with seniors 65 years of age or older eligible for dental services. 4. Income – Family income that does not exceed 150% of the Federal Poverty Level. 5. Health insurance – Inability to access affordable health insurance coverage. B. Funding Categories 1. All programs must fit into one of the Board of County Commissioners’ (BoCC) approved funding categories. The current funding categories approved for grant awards by the BoCC are (1) Behavioral Health, (2) Dental / Oral Health, and (3) Primary or Specialty Medical Care Services. C. Reimbursement Grant 6. This is a reimbursement grant. The agency must first expend its own funds and request reimbursement from the County. Allowable expenses will be reimbursed dating back to October 1, 2025.
