Contractor to coordinate and implement twenty (20) Living Well DC workshops through the Self-Management Resource Center (SMRC) for District residents with chronic diseases. The Self-Management Resource Center (SMRC) workshops will be implemented for District residents with an emphasis on the locations with the highest needs in Wards 5, 7, and 8. The SMRC workshops shall include the following: a. Chronic Disease Self Management Program (CDSMP) b. Diabetes Self-Management Program (DSMP) c. Cancer Thriving and Surviving (CTS) d. Tomando Control de su Salud (Tomando). The Contractor shall have a history of partnering/contracting with at least 2-3 DC CBOs, health providers, or agencies and should provide documentation of having worked with relevant organizations including but not limited to direct references including contact information (names, titles, phone numbers, email addresses), past contracts/subcontracts, statements of work, completion certificates, acceptance letters, reports and deliverables.
