The Wisconsin Department of Health Services (DHS) is seeking bids to supply and provide a rebate on iron-fortified infant formula purchased for the Wisconsin Special Supplemental Nutrition Program for Women, Infants, and Children (herein referred to as “WIC” or the “WIC Program”) and intends to use the results of this process to award a Contract. The selected iron-fortified milk-based liquid concentrate, powdered, and ready-to-feed infant formulas will be designated as the Department’s primary contract infant formulas authorized for participants of the WIC Program. The primary contract infant formulas will be the formulas of first choice for issuance to infants with all other formula issued as an alternative to the primary contract infant formulas. The Department does not guarantee to purchase any specific quantity or pay any minimum Contract price during the term of the Contract. Bids that require a minimum number of commodities or services be ordered will be disqualified.
