2.1 Stress test monitored: regardless of age/reviewed by cardiologist and follow- up cardiology visit if positive. 2.2 Performed by Physician/NP/PA, Review of all required documentation 2.3 Vitals- Temp, BP, P, SPO2, Ht, Wt 2.4 Complete Med/Hist/Physical Assessment 2.5 Physical Fitness Testing 2.6 Complete Urinalysis 2.7 Comprehensive Metabolic Panel 2.8 A1C, Complete Blood Count, Lipid Panel 2.9 Spirometry, Pulmonary Function Test 2.10 Vision (Depth, Near, Far, Color, peripheral) 2.11 Audiogram 2.12 Electrocardiogram 2.13 TB/PPD Skin Test 2.14 Mental health screening 2.15 Cancer screening/education, Functional medical nutrition consultation, if warranted. 2.16 Referrals generated for any medical issues uncovered during examination. 2.17 Provide blood testing to determine and document potential exposure to PFAS.
