A. Printing and packaging responsibilities: Once compatibility with TELEform software and scanner has been achieved and the English and foreign versions of the in-flight forms have been finalized and approved by DBEDT, the CONTRACTOR shall immediately provide all services required for the printing and packaging of the in-flight forms as specified below: a. English and foreign language forms shall be printed based on the following specification: Form Size: English and foreign language forms shall be trimmed to 6-1/2" X 11" Paper Quality: 24# Mocker Bond, or paper with same caliper, weight, and opacity Paper Grade: OCR grade reflective paper Paper Thickness: .0036" to .008" Paper Color: White only made from 100% wood pulp and resistant to curl *Ink Color: Face; prints black and ink read safe red** Back; prints black and ink read safe red *Ink read safe colors must have scan head compatibility readable by optical scanner. **The CONTRACTOR must also have the capability of using ink read safe purple color if requested by the STATE. b. . The CONTRACTOR shall print, package, store and deliver 3,593,520 in-flight forms from July 1, 2025 through December 31, 2025. Each carton shall have four thousand three hundred forty (4,340) in-flight forms, packaged as specified below. c. Unless otherwise requested by DBEDT, in-flight forms shall be poly wrapped in quantities of three hundred ten (310) per ream. In each ream, two hundred eighty (280) in-flight forms shall be English language forms. The remaining thirty (30) forms shall be foreign language in-flight forms in the following quantities: five (5) Chinese language forms; five (5) Japanese language forms; five (5) Korean language forms; five (5) Tagalog language forms; and ten (10) Spanish language forms. Each set of foreign language forms shall be separated. d. Include a 4” x 6” note card or note label on top of each poly wrapped ream containing the following message: “THIS PACKAGE CONTAINS 280 ENGLISH FORMS AND 30 FOREIGN LANGUAGE FORMS AT THE BOTTOM (10 FORMS IN SPANISH AND 5 FORMS EACH IN CHINESE, JAPANESE, KOREAN AND TAGALOG). PLEASE PROVIDE THESE FOREIGN LANGUAGE FORMS ACCORDINGLY UPON PASSENGERS’ REQUESTS. MAHALO!”
