Contracted vendors of Senior Outreach services will support seniors with identifying and achieving their goals to remain independent in their home and community. Services are intended to enhance a senior’s ability to maintain stability in their communities. Proposed services are to be made available throughout Hennepin County and work to reduce disparities and strengthen access to community services. Contracted vendors of Senior Outreach services will be expected to gather qualitative and quantitative data of services provided. Each proposer’s approach to collect data should incorporate senior and caregiver/family feedback into service delivery. Program outcomes for Senior Outreach services may include but are not limited to: • Quarterly reporting, detailing services provided: o Detail of how individual sessions include person-centered goals and progress towards goals; and o Detail of how group sessions include an identified learning outcome and reporting of number of seniors who achieve the identified learning outcome. • Plan for monitoring and meeting outcomes on aggregated level of data related to services provided (type, length); • Maintained or improved living situation status for seniors served; and • Senior and/or caregiver/family satisfaction is reported for services received.
