The contractor shall provide Community Option Counseling Services in nursing facilities to residents, to include identification of community resources, and to assess the individual’s appropriateness of MFP enrollment. The contractor shall provide Community Option Counseling Services to all referrals received directly from the state agency. The contractor should provide Community Option Counseling Services to at least 25% of referrals per month from the MDS Import Queue generated from the Missouri Q+ Index algorithm.
