A. Provide and attach in your response the location of proposed health care facilities including operating hours, service level capabilities, therapy available, qualifications of doctors, nurses, orthopedic specialists, therapists etc. To be labeled as Exhibit "C" Health Care Facilities. B. Medical/Physical Examinations 1. Furnish all necessary expertise and licensed medical personnel to provide medical/physical examinations for post-offer/pre- employment candidates, annual physical examinations, including a fitness-for-duty and a pulmonary test, for all City personnel who might require such examinations. 2. Perform medical/physical examinations within two business days of initial request by the City. 3. Perform medical/physical examinations for post-offer/pre- employment or fitnessfor duty candidates. Examinations could be for office staff, field (public works, water & parks, police) supervisors and field workers. If needed, the City may provide job description and requirements at the time the post-offer/preemployment examination is scheduled. Results shall be provided to the City in a format acceptable to the City. RESULTS OF EXAMINATIONS SHALL BE RELEASED ONLY TO CITY DESIGNATED PERSONS. 4. Only qualified and licensed medical practitioners shall perform medical/physical examinations required by the City. 5. Contractor shall be liable for any damages resulting from examination procedures or for reporting inaccurate results for any reason. C. Drug and Alcohol Screening 1. Provide post offer/pre-employment, Department of Transportation (DOT), anabolic steroid, and "reasonable cause" drug and/or alcohol testing with turnaround time on results as follows: 2. Negative results shall be released to City within 24 hrs after delivery of specimen to laboratory. 3. Positive results shall be released to City within Department of Transportation's ("DOT") guidelines after delivery of specimen to laboratory. 4. Provide twenty-four (24) hour-per-day back-up or on-call coverage for drug and alcohol testing collections through a combination of the facilities and its operating hours as indicated on Exhibit "C", Health Care Facilities.
