The State Department of Health seeks electronic and artificial intelligence tools to assist DOH staff and contractors in assessing individual clients’ risk of falls to allow for the development of plans to mitigate fall risks. These tools are intended for use both by public health nurses, emergency medical service providers, social workers, community health workers and other staff as well as the individuals themselves to assess gait and environmental hazards within an individual’s home which could result in the potential risk for a fall. Eligible vendors must have easy-to-use, electronic, AI driven tools which can be administered with minimal technical expertise to allow DOH staff and/or DOH contractors as well as individual clients to evaluate a person’s fall risk based his/her gait and the home environment. The tools must also provide a detailed, technical report to DOH and the client’s/patient’s primary provider as well as a non-technical report for the client. Fall risk mitigation solutions must be practical and accessible through the fall risk tool. The tool(s) must be usable from a majority of smart phones and the assessments must be able to be implemented remotely by the client(s). The vendor will work with DOH stakeholders to refine the fall risk assessment tool(s) to ensure the optimal application to a wide variety of DOH clients. The consultant will provide up to five (5) in person or virtual trainings on the use of the tools to DOH staff. The consultant will be available to DOH within 1-2 business days to help resolve technical problems that arise. The consultant shall provide the resumés of the company’s planned project team, highlighting each individual's specific experience on projects related to fall risk assessment in the last 5 years. The consultant shall be compliant on Hawaii Compliant Express (HCE) and be registered on HIEPRO (Hawaii Electronic Procurement System).
