• Contractor will conduct in-person diagnostic interviews with students to provide assessments for learning disability, ADHD, anxiety, depression, or other clinical concerns/rule outs. o For ADHD, anxiety, depression or other clinical concerns, assessment will include: Travel to the USC Upstate campus located at 800 University Way, Spartanburg Conducting the interview with the student Administering appropriate questionnaire(s) Scoring the questionnaire(s) Writing up a summary of findings Conducting a follow-up meeting with the student o For learning disability, evaluation will include: Travel to the USC Upstate campus located at 800 University Way, Spartanburg Conducting the interview with the student Administering appropriate questionnaire(s) Scoring the questionnaire(s) Administering IQ and Achievement testing Writing up a summary of findings Conducting a follow-up meeting with the student • Clinical assessments and intervention will be offered by a Doctorate level psychologist. • Contractor must be willing to complete abbreviated testing when requested. • Contractor must be licensed by the Board of Psychology. • All testing materials will be provided by the Contractor at the Contractor’s expense. • Requests for student assessment will be sent to Contractor via phone call or email from the Counseling Services Department and/or scheduled through Counseling Services on the Contractor’s Counseling Services schedule. Contractor will respond to student assessment requests within 24 hours. Contractor must schedule the student’s assessment within one week from the original request. Alternatively, Counseling Services will schedule the student under the Contractor’s schedule. • Contractor must return completed psychological evaluation to USC Upstate Counseling Services within 14 business days from the original request. • All travel expenses will be the sole responsibility of the Contractor. • Contractor must have a permanent office location in Spartanburg County, SC to ensure prompt scheduling for the in-person interview requests. • Contractor will have a minimum ten (10) years’ experience in assessing adolescents and adults. • Contractor must comply with all privacy and security measures that are required by state and or federal laws, and guidelines to include HIPAA.
