A. The Offeror shall perform medical examinations and drug testing at least four (4) days weekly, Monday through Friday, between the hours of 8:30 AM and 4:30 PM. • Potential employees will be required to complete a questionnaire, provided by the medical examiner, at the time of their appointment. The questionnaire must be approved by the City prior to the medical examiner’s use. • The medical examination shall consist of but not be limited to, examination of every body system: eyes, ears, heart, lungs, extremities, etc. Medical examinations should be appropriate based on the job title for which the potential employee/current employee is testing (refer to the appropriate job specification). B. When drug testing is required, the Company shall administer a 10-panel drug test, unless otherwise specified, and submit results within two (2) days of the test for those with an inhouse lab, or three (3) days for those who must send lab tests to an out-sourced lab. Additional time needed for specific situations will be mutually agreed upon. C. The Offeror shall agree that, additional days of medical examinations or drug testing or extended hours may be required to meet the City’s hiring deadlines. D. Specify whether you require appointments, or take walk-ins, and days and hours of operation. E. Specify whether an online portal is used for making appointments and who (i.e.- City or successful Offeror) would be responsible for making these appointments. F. If an online appointment portal is available, specify whether City liaisons would have access to the view and/or print completed medical results, drug testing results, and view steps of the workflow (i.e.- Is the potential employee scheduled for an appointment? Did they show for their appointment? Was their test submitted? Are the results back?) G. If an online appointment portal is available, specify whether potential/current employees would receive a reminder text message or email alert regarding their appointment. H. Please specify all testing locations available. I. The successful Offeror shall submit, within two (2) days of the medical exam, a form indicating whether the individual passed or failed the medical exam. Additional time needed for specific situations will be mutually agreed upon. J. The successful Offeror shall be required to submit an itemized invoice for each physical performed with the individual medical tests/drug tests listed and the cost for each test and a total for the entire medical exam per candidate
