Children and Youth Services is seeking a provider to team with the IL Supports Coordinator from Children and Youth Services to execute the majority of independent living programs and functions for the county. All independent living referrals would be initiated by the county. The provider should have a working knowledge of all the regulations and bulletins that govern independent living in the Commonwealth of Pennsylvania as well as the state and federal funding sources. The provider should be innovative and flexible in their delivery of services. The provider must have a diverse and culturally competent team who have knowledge and experience working with this population. The Independent Living provider would provide essential services to Independent Living youth. The provider will work with the assigned Children and Youth Services caseworker and supervisor to ensure all youth who meet eligibility are offered independent living services. Outreach to the youth and services provided should be timely and address the specific needs of the youth. These services would include, but are not limited to: • Evaluate youth eligibility for different categories of independent living services. • Utilize the Ansel-Casey Assessment tool for all youth within forty-five days of the referral or within 45 days of a youth turning fourteen. If a youth enters out of care after the age of sixteen or a youth over the age of eighteen requests resumption of dependency and re-enters care, the Ansel-Casey must be completed in thirty days. • Develop, in conjunction with the youth’s casework team, an independent living plan that incorporates the results of the Ansel-Casey Assessment. These services could include life skills, prevention services, education and training, employment, support, housing, physical and mental healthcare. The provider would update the plan every six months.
