The Queen Anne’s County Department of Health (QACDOH) seeks an organizaon to assist with mulple healthy cooking demonstraons and providing a full meal for the Diabetes Prevenon Program at each event, locaons and dates to be determined. While most events will not exceed 30 parcipants, the exact number of parcipants for each event will be provided to the vendor 7 days prior to the event. The vendor will give a live, in person presentaon of a healthy cooking demonstraon and serve a meal to the parcipants in aendance. These cooking demonstraons are to encourage and educate our community about healthy and nutrious eang that promote a healthier lifestyle. Services to be Performed/Statement of Work/Deliverables; A. Submit a menu to the Queen Anne’s County Health Department one week prior to the event, for approval; B. Provide a diabec friendly, healthy cooking demonstraon lasng approximately one (1) hour at each event; C. Provide a full diabec friendly, healthy meal, including beverages, for a predetermined number of parcipants; D. Supply and set up all food related items; E. Provide disposable plates, utensils, napkins, bowls, cups, etc. F. Provide take out boxes for parcipants to pack any leover food items; G. Clean up all food related items; H. Submit experience and qualificaons to provide presentaons on healthy cooking; I. Must be insured and bonded; J. Must provide the organizaon’s Federal Idenficaon Number (FEIN) & W-9.
