● Work in concert with MOH to develop a plan that ensures that those homeless households prioritized for assistance through Boston’s Coordinated Entry System have access to BRAF, no matter the shelter they reside in or whether they are experiencing homelessness on the streets. ● Provide eligible homeless households with necessary financial assistance to facilitate their access to permanent housing. ● Issue payments to participating property owners in accordance with program regulations and contract requirements. ● Conduct outreach in such a way as to ensure that BRAF funds are accessible to all Boston shelters and is inclusive of all racial, ethnic and linguistic groups. ● Coordinate BRAF activities with other agencies/ programs that are providing similar services and refer clients to additional services when appropriate. ● Maintain a file for each client who receives services from the BRAF program. Each file shall contain detailed, thorough and organized records. The files must be available for regular monitoring visits by MOH. ● Participate in an approved Homeless Management Information System (HMIS). Keep computerized records of the households served by the program (e.g., chronic homeless and homeless status, income, race, ethnicity, gender, age, household size, type and location of permanent housing placement, and outcome of assistance). ● Submit an annual quantitative progress report and other periodic reports of BRAF-funded program activities, as required by MOH. ● Establish reasonable performance objectives and then track, measure, and report on said objectives in an annual quantitative performance report to MOH. Participant status must be tracked for a minimum of one year after receipt of BRAF assistance. ● Submit requisitions for reimbursement of program expenses to MOH on a monthly basis. All requisitions need to conform to MOH requirements, be submitted on the appropriate forms and contain complete and properly ordered backup documentation for requisitions to be paid in a timely fashion. ● Indicate how BRAF coordinates with other rapid rehousing resources in the City. Please include all rapid rehousing resources whether managed by the applicant or by other agencies. ● Pursuant to 2 CFR 200.501, a Contractor receiving more than $750,000 in Federal assistance in a fiscal year and qualifying as a subrecipient, must meet the requirements of 2 CFR 200, Subpart F, “Audit Requirements.”
