o Provide on-site, weekly 2-hour group classes focused on target population for 14 weeks o Work and collaborate with Prince George’s county Department of social Services PGCDSS) to screen each referral to assure the appropriateness of the referral. o Ensure all staff facilitating the SFP process has successfully completed the SFP training. o Provide incentives, food, babysitting, and transportation to facilitate engagement to all families participating in the SFP. o Collect pre and post data to evaluate SFP process. o Establish a process to record attendance for fourteen (14)-week session. o Provide linkage and referral for any additional services families may require. o Report any unusual incidents to PGCDSS’s representative within twenty-four (24) hours of incident. o Adhere to all necessary guidelines to assure services are implemented and fidelity.
