i. Perform inspections of all conveyances except those in or attached to single-family homes; 1. Inspections shall be performed in accordance with the Illinois Compiled Statutes 225 ILCS 312 and the rules and regulations of the Office of the State Fire Marshal of the State of Illinois; 2. For each conveyance inspected which did not completely pass the inspection, provide a copy of a Field Correction Notice showing the date, address, Village of Lisle elevator number, state registration tag number, and description of those items not meeting current conveyance code requirements; 3. Upon request of the Village of Lisle, re-inspect any conveyances for which field correction notices were issued, and report the results of the inspection; ii. Review and approve plans and specifications for installation of new conveyances within the Village of Lisle. The contractor shall complete all plan reviews of new equipment within 10 working days following receipt; iii. Inspect the installation of new conveyances and approve or reject their use; provide written justification for rejection, and provide written confirmation of approval for use; iv. Provide guidance and advice to the Village of Lisle upon request regarding technical requirements, code changes, interpretation of state standards and rules; v. The contractor shall respond to emergency call-outs within one hour by phone, and be on-site within four hours upon notification of an emergency. The contractor shall provide the Village a twenty-four hour point of contact, to include name and phone number that will be able to meet these requirements; vi. The contractor shall be required to witness various periodic conveyance tests, as required by state statutes and adopted elevator codes. Contractor will bill the owner’s elevator maintenance company directly for the witnessing work. B. Village of Lisle will carry out Service in accordance with the following: i. Provide to the elevator inspection contractor annually a current list of elevators within the Village of Lisle, including the Village of Lisle conveyance license number and the state registration tag number for each; ii. Upon receipt of each annual inspection report from the elevator inspection contractor, send elevator certificates to those conveyances that passed the inspection; send violation letters to those elevators for which field correction notices were issued. The Village of Lisle will follow up on violations until compliance is achieved; iii. Issue elevator certificates to new conveyances upon approval for use by the elevator inspection company.
