3.1 Background The County has constructed office/retail space suitable for a pharmacy within the Ricard Hall complex. The space is designed to be a secure space with controlled access and equipped to accommodate the needed storage, IT and computers required by the Pharmacy. The County has established a minimum rent amount for the initial term of the contract. 3.2 Intent Somerset County is soliciting proposals for on-site pharmacy services to provide high quality, personalized services to individuals who are clients of Richard Hall Community Health and Wellness Center (RHCHWC). The Contractor shall establish, manage, and operate on site at the Richard Hall Community Health and Wellness Center and shall provide, at minimum, the following services: • Dispense prescription medication • Assist clients in obtaining the best rates for their prescription drugs • Provide Prescription Drug Assistance/Discount Drug Program • Provide Medication counseling/education • Provide Medication counseling for diabetics • Provide Medication counseling for Ryan White patients • Provide Medication reconciliation for patients • Provide no cost prescription delivery to include mail delivery • Complete all prior authorizations • Pre-filled daily dose medication packaging • No-cost dispensing fees • Medication utilization and analysis • Dashboard to track patient adherence • 24/7/360 customer service line • Refill management/reminder calls and medication synchronization • Clozapine Monitoring
