Specifications include, but are not limited to: Project application and proposed consultant services must address: • Framework, approach, methods, and timelines (including a draft Recruitment Guide, subject to modification in consultation with Community Health Improvement (CHIP) Leadership Team and relevant CHIP Implementation Action Teams) for recruiting approximately 75 representative persons to serve on 5 to 6 focus groups: from urban locations in the City of St. Louis and St. Louis County with incomes of 300% or less of Federal Poverty Level; who live in areas where health data show underperforming health mortality and morbidity rates; diverse ages (younger and older); Male and Female; who have physical or developmental/mental disabilities; From diverse background and cultures; who speak a first language other than English, which may limit their access to health information; who are in communities that may need additional resources to achieve health results equality with State and National health results averages; and/or have lived experience as persons in communities that have experienced reduced levels of services necessary for improved health; higher than State or National averages of violence; behavioral health issues (including substance use disorders); low access to affordable, accessible and culturally acceptable nutrition; higher than State or National averages for chronic disease and/or health challenges.
