This is considered a single source purchase because The Ages and States Questionnaires are registered trademarks available only through Brookes Publishing. Minnesota’s Follow Along Program uses the Ages and States Questionnaires (ASQ) as recommended by Minnesota’s Developmental Screening Task Force, a partnership of the Minnesota Departments of Education (MDE), Health (MDH) and Human Services (DHS) (Minnesota Statutes 121A.17 Sub 3 (b).
