1. Provide laboratory diagnostic testing and phlebotomy services for patients on the islands of Oahu, Maui, Hawaii, and Kauai. Services shall include pickup of specimens from various medical providers' offices on the islands listed above, drawing of specimens by the Bidder at the Bidder’s designated laboratory and phlebotomy sites, and delivery of completed laboratory test results. 2. Perform each test listed in the attached List of Laboratory Tests for tests that the Bidder is not able to perform in its own facility, the Bidder may subcontract with a reference laboratory or other laboratory provider, provided that the technical quality and accuracy of the test shall be maintained. 3. Provide all required supplies and materials to perform the requested services. The Bidder shall also provide, at no additional cost to the HRSB, any repeat tests and supplies for laboratory specimens that require special handling or special shipping precautions.
