Professional assessment in order to diagnose or rule out neurological disorders by assessing: • Reviewing current academic performance when available • Mental status • Motor skills • Sensory skills • Balance and coordination • Reflexes • Functioning of the nerves • Participate in IEP meetings to discuss findings upon request; Submission of Completed Evaluations and/or Reports: • All evaluations and reports will be sent electronically to the requestor as well as the designated contact in the Department of Special Education via email; • All evaluations and reports are to be signed, scanned & attached to the email as a PDF file; • All testing materials should be attached to a hard copy and mailed to the requestor, not to the Department of Special Education; • All Evaluations are to be completed and provided to the requester no later than 14 days after assignment receipt...
