The Contractor must provide ice merchandiser freezer filled to capacity with either 7 lb., 10 lb., or 20 lb. bags of Ice. Merchandiser freezer may be new or used, shall be clean, in efficient operating condition, and in compliance with State and Federal laws, regulations, and health standards. Vendor is to complete blanks below for each type of container bid. Required power supply 110 Volts Pounds per Bag Number of Bags per Merchandiser Total Pounds of Ice per Merchandiser Merchandiser Dimensions Price per pound of cubed ice is to include use of merchandiser provided by the Contractor. The Contractor will be responsible for delivery and pickup of merchandisers. The State of Louisiana will not be responsible for maintenance or operation of merchandisers. Minimum order for this line is one full merchandising unit upon initial delivery to site. Vendor will refill or replace merchandiser thereafter with quantities of ice ordered. LB 1700 Line 2 Daily Rental Rate for Merchandiser with Ice: Rate that will be charged daily for the merchandiser with bagged ice. The day, for billing purposes, will begin when the merchandiser has been delivered and set up at the Agency site. The day will be 24 hours from time of delivery/set-up. If the merchandiser has been in the possession of the State for 7 days or longer, the daily rates will not apply, the weekly rate will be used. See Line 3. Example: 9 days = 1 Week and 2 days DAY 1 Line 3 Weekly Rental Rate for Merchandiser with Ice: Rate that will be charged weekly for the merchandiser with bagged ice. If the merchandiser is in the possession of the State for 7 days or longer, the daily rates will not apply, the weekly rate will be used. Reefer trailer may be used or new, shall be clean, in efficient operating condition and in compliance with State and Federal laws, regulations and health standards. The Contractor will be responsible for delivery and pickup of these trailers. The Contractor will be responsible for providing a full fuel tank for self-contained powered reefer units upon initial setup on site. The State will refuel the reefer trailer(s) as needed thereafter while in the State's possession. Minimum order is one full truck/reefer trailer upon initial delivery to site. Vendor will refill or replace with full reefer trailer thereafter containing quantities of ice ordered. For billing purposes, this line is for any ice used from a reefer trailer or if the seal to the door has been broken. Vendor Can Only Charge for line 4 or 11, Not Both LB 42,000 Line 5 Daily Rental Rate for Reefer without Driver: Daily Rates: Rates that will be charged daily for the reefer trailer containing bagged ice. The day, for billing purposes, will begin when the driver has checked in with the Agency. The day will be 24 hours from check in time.
