The purpose of this solicitation (hereinafter the “Solicitation”) is to obtain the services of a group vision plan (hereinafter the “Contract” or “Vision Plan”) for eligible active State and university employees, retirees, COBRA members, and their eligible dependents (hereinafter, “members” or “participants”). 1.3. The State intends to award a Contract for a fully insured vision plan. The State seeks a contractor that shall collaborate directly with ADOA and other ADOA-contracted vendors at the request of ADOA to fulfill the Contract. 1.4. The State has provided a fully-insured vision benefit for all eligible members through Avesis, Inc. since 2008. The contract number CTR049918 available in Arizona Procurement Portal at https://app.az.gov/. 1.5. Contractor shall be required to begin services prior to the Plan year effective date in order to prepare for implementation of the Vision Plan (e.g., open enrollment). The contractor who is awarded this Solicitation will begin implementation of their services immediately following the award in preparation with a targeted “go live” date of January 1, 2026. Services under the awarded contract are for three (3) complete plan years with two (2) optional plan years dependent on contract extensions.
