The Tennessee Department of Health (TDH) is soliciting proposals to support the Centers for Disease Control and Prevention (CDC), (Division of Heart Disease and Stroke) mission to promote and create a SelfMeasured Blood Pressure (SMBP) Monitoring Platform to provide clinical support among populations at highest risk of hypertension. The State is requesting a vendor who will develop and implement a web-based, HIPAA-compliant SMBP and clinical support platform that will be made available to all adult Tennesseans and healthcare providers licensed to practice medicine in Tennessee through various clinical sites, including but not limited to local health departments, private practice, or other external clinical/medical settings where patients are referred to the SMBP platform. The SMBP Platform must provide clinical support which includes: regular one-on-one counseling, web-based or telephonic support tools, and access to educational classes/resources. In addition, the SMBP platform will require use of Blood Pressure Measurement Devices, which must be included in the proposal to be distributed to patients for SMBP. The Blood Pressure Measurement Device must be validated for clinical accuracy according to the United States Blood Pressure Validated Device standards.
